Writing Orders for Medications

Physicians are reminded that orders should not be written after the Physician's Order Form has been processed.

Please make sure the yellow copy of the order sheet has not been pulled when writing orders for medications. The Pharmacy Dept. fills orders based on the yellow copy of each Physician's Order Form.

A recent copy had an order for Ativan 1mg; however, orders for Aricept, Lipitor, Afrin nasal spray and Elidel cream were written after the yellow copy was pulled. The pharmacist fortunately saw the original order sheet and the orders were processed. A similar situation happened when an original order was written to add prednisone and discontinue Dilaudid. Additional orders written after the yellow copy was pulled included: discontinue Ambien and Tylenol ATC, and start Ativan. These orders were not carried out until discovered at a later date.
