Include INDICATIONS in Order "Per Pharmacy"

When writing "per pharmacy protocol" orders, the Pharmacy Dept. asks that physicians please remember to include the indication for the requested medication (e.g. vancomycin for osteomyelitis, warfarin for atrial fibrillation, etc.). The indication is essential in order to ensure the appropriate dose, duration, target drug levels and monitoring of the drug for patients. The pharmacist needs to know this information to accurately process orders and ensure that patients receive the appropriate treatment for the intended indication. This will also help avoid unnecessary calls to physicians and prevent any delays in delivering the medication to patients.
