CHF Discharge Medication Compliance

CHF Discharge Medication Compliance Reaches 70 Percent

CHF (Congestive Heart Failure) Discharge Medication Compliance reached 70 percent in February 2007, representing a significant increase from this time last year when the compliance rate was only 30 to 40 percent.

Congratulations and thanks to our 70-plus physicians who are compliant with the CHF Discharge Medication Core Measure. Click here to view the list of physicians who have complied.

As a reminder, a two-step process is required by physicians discharging Congestive Heart Failure patients:

  • 1) Completion of the E-Discharge Medication List (available in Web/VS) with signature;
  • 2) Dictation of the exact same list of medications.
Physicians are advised to dictate their discharge summary at the time of discharge using the list. A scanned version of the list will be available in the medication tab of Web/VS after discharge. To be compliant with the JCAHO/CMS measure, the E-Discharge Medication List, which the patient receives, and the medication list in the dictated summary must match.

The E-Discharge Medication List is mandatory for CHF patients and highly recommended for all others.
