The Web/VS Tip of the Week

Web/VS Interactive Forms

Web/VS supports a number of different forms that allow clinicians to send questions or comments to support staff or administration, request films or images, and (for full-time staff) submit encounter reports.

The different types of Interactive Forms are as follows:

  • Contact Us - The Contact Us form is activated by the Contact Us button in the lower left corner of the patient search screen. The form can be used to send a complaint, question, or suggestion directly to the Web/VS support team.
  • MD Feedback - The MD Feedback form is activated by the MD Feedback button in the lower left corner of the patient results screen. The button is only available for users with the MD role and Attending rank. The form can be used to send a patient safety or patient care issue, complaint, suggestion, or other information directly to the hospital's Quality Improvement team. The information is submitted under 1157 protection.

  • Ask Administration - The Ask Administration form is activated by the Ask Administration button in the patient search screen. The button is only available for users with the MD role. The form can be used to send a question, suggestion, comment or complaint directly to the hospital's senior executives.
  • Film/Image Request - The Film and Image Request form is activated by a Film/Image Request button that is displayed at the bottom of every imaging report. The form can be used to send a request directly to the imaging report for a copy of a film or image or to have an image pushed to the PACS web server.
  • Contact a Radiologist - The Contact a Radiologist form is activated by a Contact Radiologist button that is displayed at the bottom of every imaging report. The form can be used to send a question, comment, suggestion, problem or complaint to the Imaging physicians.
  • Release of Information - The online Release of Information form is activated by the ROI button in the lower left corner of the patient search screen. The user is prompted to renew the online ROI on a yearly basis. If the user clicks the "I agree" button, the user can view their own lab results and reports (with certain restrictions) in Web/VS, otherwise, such accesses are blocked. Copies of the ROI are mailed to the user and archived in a public folder.
