Dalteparin (Fragmin) per Pharmacy Protocol for Treatment of DVT/PE

The Department of Pharmacy Services now offers a Pharmacy and Therapeutics Committee approved LMWH protocol for the treatment of DVT/PE as an alternative to unfractionated heparin infusion in appropriate patients.

Inclusion criteria include: treatment of PE/DVT only, ClCr greater than 30ml/min, and no invasive procedure planned within 24 hours (if dosed every 24 hours) or within 12 hours (if dosed every 12 hours). Invasive procedure is defined as any procedure other than the following: dental extraction, cataract surgery and dermatological surgery.

Exclusion criteria include: invasive procedure planned within 24 hours, ClCr less than 30ml/min, documented hypocoagulable state (i.e., elevated aPTT, PT/INR at baseline, thrombocytopenia), suspected DIC, CVA/atrial fibrillation, mechanical valve replacement or history of HIT).
