New Codes Issued for Advance Care Planning

The Cedars-Sinai Quality Council has identified improving end-of-life care as a high priority for this year. Areas of focus include improving advance care planning and documentation of patient goals and values for healthcare.

The federal Centers for Medicare & Medicaid Services have issued two new Current Procedural Terminology codes (99497 and 99498) to reimburse physicians for their time performing advance care planning. These new codes are time-based, with 99497 to be used when billing for the first 30 minutes of the discussion, and 99498 used for each additional 30 minutes.

These codes can be used in addition to your normal evaluation and management visit codes, as long as you use the "-25" modifier and document the time spent on the conversations about advance care planning. This time needs to be separate from, and in addition to, the time spent on the normal visit.

Documentation requirements for advance care planning discussions must include the following points for reimbursement:

  • Designation of a person who will make decisions for the patient if the patient cannot speak for himself or herself
  • The types of medical care preferred
  • The comfort level that is preferred
  • An indication of whether an advance directive or Physician Orders for Life-Sustaining Treatment document has been completed

In CS-Link, documentation of advance care planning discussions should be done using the new advance care planning note type. These notes are viewable in the Chart Review activity, in both the "Notes" tab and in the new "Adv Care Plan" tab. The note can be created by selecting "Advance Care Planning" in the "Type" field.
