New Requirements for OR and Procedural Areas

There is a new requirement in all OR and procedural areas for surgeons and proceduralists to complete SEPARATE attestation and interval H&P notes prior to starting a case.

To support this new workflow, there will be changes in CS-Link™ to help improve efficiency and assure compliance with regulatory requirements. We encourage everybody to use the available "Surgical" and "Procedural" navigators to walk you through these steps in CS-Link.

The CS-Link changes include the removal of the interval H&P statement from the .PREATT smart phrase and the creation of a new interval H&P workflow that leverages the ".INTERVALHP" smart phrase and note type.

IMPORTANT: If you have your own personal attestation smart phrases, please remove the language about the interval H&P and make sure to complete a separate note.

As a reminder, each document has the following requirements:

Pre-Op / Pre-Procedural Attestation:

  • .PREATT smart phrase
  • Pre-Op (Attestation) note type
  • This note confirms that consents for the proposed procedure and potential blood transfusion have been obtained
  • The note be documented any time after the discussions have taken place, and must be documented by the physician performing the surgery or procedure

Interval H&P:

  • .INTERVALHP smart phrase
  • Must be documented within 24 hours of the surgery or procedure and AFTER the patient has been admitted

We also have increased our focus on the importance of completing a brief post-op/post-procedural note prior to each patient leaving the OR or procedural suite. There will be increased reminders from nursing staff and signs posted around the ORs and procedural areas to remind surgeons and proceduralists to complete this note in a timely manner.

Please remember to use the "Post-Op (Brief Op Note)" note type and the .POSTATTEST smart phrase, which contains all the necessary elements, when completing this note.
